Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one in-home and senior living care in Warren/Basking Ridge/Watchung, NJ. You’ll evaluate, plan, and apply evidence-based therapy to help older adults improve mobility and independence, with scheduling autonomy and optional telehealth integration.

The role emphasizes patient relationships, flexible schedules, and a clinician-led culture, with access to advanced tools and career development opportunities.

Qualifications

• Active Physical Therapy license in the state(s) of practice, or eligibility to obtain a degree from an accredited Physical Therapy program.
• Current CPR certification.
• Ability to perform in-home and senior living care with flexibility.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes and senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility, independence and quality of life.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
