Job summary

Seva Home Health in Madera, CA is seeking a licensed Physical Therapist to provide one-on-one home health therapy across Fresno, Madera, Tulare and Kings counties. Weekly mileage reimbursement and paid driving time are provided.

Positions available as part-time and full-time, with a supportive, friendly environment and flexible schedule. You will assess and treat a variety of diagnoses in the patient’s home setting, tailoring plans to individual goals while maintaining high professional

Qualifications

• 1 year of experience as a Physical Therapist.
• Current California license to practice Physical Therapy.
• Current CPR/BLS certification.
• Reliable transportation.
• Compliant with state healthcare regulatory requirements.
• Knowledge of PT treatments and home health programs.
• Ability to tailor plans to individual patients.
• Strong observation and communication skills; basic math.

Responsibilities

• Initiates PT evaluation and develops care plans for home health patients.
• Educates patients and families on prosthetic device use when appropriate.
• Prepares and submits progress summaries using appropriate documentation.
• Participates in start-of-care and discharge planning.
• Provides PT consultation to families and caregivers as needed.
• Offers in-service education for nursing staff when required.
• Participates in interdisciplinary group and quality improvement activities.
• Supervises physical therapy assistants.
