CVHCare is seeking a dedicated Physical Therapist to join our home health team serving San Ramon, CA and surrounding areas. You will evaluate mobility, strength, and functional level in patients' homes, establishing goals to improve balance, walking, and independence.

Qualifications

• Licensed Physical Therapist in California with current license.
• BLS certification required or active.
• Proficient with Windows/Microsoft Office and email/G Suite.
• Strong documentation and charting skills.
• Ability to work independently in patients' homes.
• Experience in home health is a plus but not required.

Responsibilities

• Evaluate mobility, strength and functional level in the home setting.
• Set goals to improve balance, gait, strength and function.
• Educate patients and caregivers on safe mobility and use of equipment.
• Assess home environment for safety and recommend assistive devices.
• Coordinate with doctor and patient support system to ensure continuity of care.
