Job summary

Seva Home Health seeks a licensed Physical Therapist for our Fresno service area. Both Part Time and Full Time positions are available, with mileage reimbursement and paid driving time for visits across Fresno, Madera, Tulare and Kings Counties.

Qualifications

• 1 year of experience as a Physical Therapist.
• Must have a current and unrestricted California PT license.
• CPR/BLS certification is required.
• Proficient with electronic medical records (Kinnser/WellSky a plus).

Responsibilities

• Perform PT evaluations and develop care plans for home health patients.
• Educate patients and families/caregivers in therapy techniques.
• Complete clinical and progress notes using EMR and OASIS as needed.
• Coordinate with nursing and therapy staff for discharge planning.
• Provide supervision to therapy assistants when applicable.
• Participate in quality improvement activities.

Skills/Requirements

• 1 year of experience as a Physical Therapist
• A current and unrestricted license to practice Physical Therapy in the State of California.
• Current CPR/BLS certification.
• Reliable transportation
• Compliant with state healthcare regulatory requirements.
• Possess up-to-date knowledge of treatment practices and physical therapy programs.
• Ability to care for patients with different personalities.
• Ability to assess patient condition and work with treatment plans.
• Strong skills in observation, verbal and written communication, problem-solving, basic math, Electronic Medical Record Software (Kinnser/Wellsky a plus), and basic computer skills to manage email, texts, employment record self-service, etc.

Essential Job Functions/Responsibilities

• Initiates physical therapy evaluation and care plan and instructs other personnel and/or family/ caregivers in certain phases of physical therapy with which they may work with a patient, focusing on patients therapeutic rehabilitation.
• Educate and prepare patient for use of prosthetic devices
• Prepares and submits clinical and progress summaries utilizing OASIS for all documentation
• Participates in patient start of care and discharge planning.
• Provides physical therapy consultation to families/caregivers, as needed
• Provides in-service education programs for nursing personnel as needed.
• Participates in the interdisciplinary group.
• Actively participates in the quality assessment performance improvement teams and activities.
