Job summary

Sonder Healthcare is seeking a licensed Physical Therapist to provide in-home, patient-centered care across Tracy, Stockton, and Manteca, CA. This full-time role involves evaluating patients in their homes, developing personalized treatment plans, and guiding PTAs as part of an interdisciplinary team.

The position offers a per-visit pay structure with flexible scheduling and on-the-road work. A valid California PT license, CPR/BLS, and reliable transportation are required; home health experience

Qualifications

• Must hold an active Physical Therapist license in the state of employment.
• Strong assessment and treatment planning skills.
• Ability to pass background check and screenings.

Responsibilities

• Conduct initial patient visits, including comprehensive assessments and evaluations
• Develop and implement individualized treatment plans
• Supervise and collaborate with Physical Therapist Assistants (PTAs)
• Coordinate care with interdisciplinary team members
