Job summary

BrightStar Care – Concord, Lexington & Woburn is seeking a licensed Physical Therapist to provide in-home care on a part-time/per diem basis. You’ll assess clients in their homes, design personalized plans, and help restore strength, balance, and independence.

You’ll work with a multidisciplinary team, use a mobile EMR, and receive competitive pay ($75–$85/hour) with mileage reimbursement. Flexible scheduling and opportunities for training and CE support are available.

Qualifications

• Licensed Physical Therapist with MA license.
• DPT or MPT from an accredited program.
• CPR certification.
• 1+ year of experience preferred; new grads welcome.
• Reliable vehicle and valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
