SHC WVUHS Home Care is seeking a licensed Physical Therapist to evaluate, treat, and progress patients in a home care setting in the United States. The role includes consultation with medical staff, documentation, and supervision of assistive personnel.

Qualified candidates possess a current PT license in the applicable state, CPR certification, and are able to travel to patient locations. The position emphasizes comprehensive patient care and adherence to standards and policies.

Qualifications

• Current physical therapist licensure in the applicable state where services will be performed.
• MD: PT license through Maryland Board of Physical Therapy Examiners OR PT Compact License
• OH: PT license through Ohio OT, PT, and AT Board OR PT Compact License
• PA: PT license through PA State Board OR PT Compact License
• WV: PT license through WV Board OR PT Compact License
• Valid driver’s license.
• CPR certification within 30 days of hire date.

Responsabilités

• Performs evaluation procedures according to policy and standard of care (musculoskeletal/orthotest, neuro/motor test, gait analysis, manual muscle test, functional analysis, prosthetic/orthotic evaluation).
• Deliver and analyze physical therapy treatment (modalities, exercise, manual skills).
• Implements home programs and educates patients/families per procedures.
• Supervise assistive personnel and PT students.
• Provide care considering age-specific needs for infants to older adults.
• Communicate patient status with home care team and at staff meetings.
• Document supervisory visits per standard; complete paperwork timely.
• Monitor staff compliance with safe environment; perform CPR as needed.
• Apply/maintain equipment per policy; manage supply requisition.
