WVUHS Home Care in Elizabeth, WV seeks a licensed Physical Therapist to evaluate, plan, and deliver patient-focused physical therapy in the patient’s home. You will collaborate with medical staff, complete documentation, and educate patients and families.

The role requires active state licensure, CPR within 30 days, and a valid driver’s license. Travel to patient homes is a regular part of the role, with standard 40-hour weeks.

Competenze

• Active Physical Therapist license in applicable state.
• CPR certification within 30 days of hire.
• Valid driver’s license.
• Eligible for APTA or WVPTA membership.

Mansioni

• Perform evaluation and treatment procedures per plan of care.
• Deliver and monitor physical therapy treatment and modalities.
• Develop and adjust home exercise programs; educate patients and families.
• Supervise PTAs and maintain supervision records as required.
• Coordinate with home care team to adjust care levels as needed.
• Document patient progress and complete required paperwork promptly.
