Well Care Health is seeking a compassionate Home Health Physical Therapist to deliver high-quality therapy to patients in their Durham County, NC area. You will work under supervision to develop and implement individualized treatment plans and coordinate care with a multidisciplinary team.

The role requires graduation from an accredited Physical Therapy program, NC/SC licensure, CPR certification, and at least 1 year of experience.

Qualifikationen

• Graduate of Physical Therapy program from an accredited university.
• Licensure in NC or SC, CPR certification.
• Minimum 1 year of experience.
• Ability to transfer and guard patients, use modalities and assistive devices.

• Provide home health physical therapy services to patients in their homes.
• Assess, plan, implement, and evaluate patient care with age-related considerations.
• Collaborate with care team; educate patients and families on management and self-care.
• Maintain accurate documentation per agency policies and procedures.
