Duke Health – HomeCare & Hospice in Durham, NC seeks a Physical Therapist to deliver therapeutic services in home and inpatient hospice settings. The senior therapist guides staff, develops care plans, and ensures evidence-based practice.

You will partner with physicians and the care team to optimize functional outcomes, implement age-appropriate plans, and support patient/family education and discharge planning. Experience and NC licensure compliance are required.

Qualifications

• Ability to push-pull, lift-carry, and transfer patients.

Responsabilités

• Provide physical therapy services to patients with impairments or functional changes.
• Develop and update plans of care with measurable goals and discharge plans.
• Collaborate with physicians, nursing, and rehab staff to coordinate patient care.
• Conduct patient and family education and ensure understanding of interventions.
• Document findings and ensure timely, accurate charting.
