Job summary

JourneyCare Home Health is seeking a skilled Home Health Physical Therapist to cover Hyde Park - Chicago, IL and surrounding areas. You will provide PT services in patients' homes, creating and executing treatment plans to improve mobility and quality of life.

The role requires home health experience, a PT license, and at least two years of acute care experience. Full-time or PRN options are available with competitive salary and strong benefits.

Qualifications

• Graduate from a School of Physical Therapy approved by the American Physical Therapy Association and Allied Health Education and Accreditation of the AMA and the Physical Therapy Association.
• Current licensure as a Physical Therapist in the state of practice.
• At least two (2) years’ experience in an acute hospital or equivalent experience (preferred).

Responsibilities

• Provides physical therapy services to patients in their homes according to a physician's plan of care and state practice act.
• Advise, consult and teach appropriate treatment procedures to clients, family members and agency staff
• Assist the provider in evaluating level of function
• Participate in development of the plan of care
• Prepare clinical and progress notes
• Translate exercises into functional activities or activities of daily living
• Develop plans for modifying equipment and environment in clients' homes
• Communicate progress and factors affecting care to the provider and family
