Trinity Health Home Care is seeking a dedicated Physical Therapist to visit patients in their homes across a broad service area in Illinois. The role emphasizes one-to-one patient attention, autonomy, and using advanced technology to deliver compassionate, high-quality care.

Qualifications

• Bachelor’s degree with progressive experience or Master’s degree.
• Licensed to practice as a physical therapist in Illinois.
• Current working knowledge of conditions of participation and home healthcare experience preferred.
• Must have current driver’s license and reliable transportation to and from work site.
