Job summary

JourneyCare Home Health in Chicago’s Hyde Park area is seeking a skilled Home Health Physical Therapist to provide in-home therapy, develop effective treatment plans, and improve patient mobility and quality of life.

You will deliver PT services per physician plans, educate patients and families, evaluate function, prepare notes, and supervise PT assistants while maintaining accurate records.

Qualifications

• Graduate from a School of Physical Therapy approved by the American Physical Therapy Association.
• Current licensure as a Physical Therapist in the state of practice.
• At least two years of acute hospital or equivalent experience (preferred).

Responsibilities

• Provides physical therapy services to patients according to a written physician's plan of care and state regulations.
• Advise, consult and teach appropriate treatment procedures to clients, families and staff.
• Assist the provider in evaluating level of function.
• Participate in the development of the plan of care.
• Prepare clinical and progress notes.
• Translate exercises into functional activities.
• Develop plans for modifying equipment and home environment.
• Regularly report progress and factors affecting care to the provider.
