Loyola Medicine Home Care and Hospice is seeking a Physical Therapist to provide in-home PT services in the Berwyn, Broadview, Riverside areas and the South Side of Chicago. You will join a multidisciplinary team delivering personalized care to patients at home, with autonomy and support in a mission-driven environment.

Qualified candidates hold a PT degree, Illinois license, and have a valid driver’s license; home healthcare experience is preferred.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy.
• Licensed to practice in Illinois.
• Home healthcare experience preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver physical therapy services based on a personalized care plan.
