Job summary

FOX Rehabilitation is recruiting licensed Physical Therapists to deliver one-on-one geriatric care in private homes and senior living communities in the Dubuque, IA area. You will evaluate, plan, and deliver evidence-based therapies, improving mobility and independence for older adults.

The role offers autonomy, flexible scheduling, and opportunities to incorporate telehealth where appropriate to extend access and continuity of care.

Qualifications

• Valid Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in-home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and quality of life.
• Document care effectively in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
