Job summary

Connectly Recruiting is seeking a passionate Physical Therapist Assistant to provide in-home care in San Jose, CA. You will assess mobility, design personalized treatment plans, and collaborate with a multidisciplinary team to restore function and independence for patients.

The ideal candidate holds a California PTA license, active BLS/ACLS, up-to-date immunizations, a negative TB test, and a valid driver’s license to travel to clients’ homes. Patient-centered care and safety are prioritized.

Qualifications

• Current Physical Therapist Assistant license in California.
• Valid CA Fingerprint Clearance Card.
• Up-to-date BLS (CPR) / ACLS certification.
• Recent Negative TB test result and Flu Shot within the past year.
• Immunization records including Covid19, Hep B, MMR, TDaP, Varicella.

Responsibilities

• Delivering skilled PT interventions to improve mobility and function in patients' homes.
• Creating and executing individualized treatment plans.
• Collaborating with a multidisciplinary healthcare team for holistic care.

Licensed PTA CA

BLS/ACLS certified
