Job summary

Zaddy Solutions is seeking a dedicated Physical Therapist Assistant to provide high-quality, in-home physical therapy in San Jose, CA. You will assess and treat patients in their homes, designing personalized plans to improve mobility and daily function.

The role requires an active CA PTA license, current CPR/ACLS, TB test and annual flu shot, and up-to-date immunizations. A valid driver’s license and insurance are also required to reach patients' homes within the service area.

Qualifications

• Current Physical Therapist Assistant license in the State of California.
• Valid CA Fingerprint Clearance Card.
• Up-to-date BLS (CPR) / ACLS certification.
• Recent Negative TB test result and Flu Shot within the past year.
• Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.

Responsibilities

• Delivering skilled physical therapy interventions, including exercises and stretches, aimed at enhancing patients' mobility and function.
• Creating and executing individualized treatment plans tailored to each patient's specific needs and goals.
• Collaborating with a multidisciplinary team of healthcare professionals to ensure holistic and coordinated patient care.
