Job summary

Home Health Care, Inc. is seeking a Licensed Physical Therapist Assistant to provide in-home physical therapy across the western metro and Buffalo-area territory. The PTA will deliver one-on-one treatments, implement plans of care, and collaborate with a multidisciplinary team to help patients regain independence.

The role emphasizes autonomy in the field, with travel to patients’ homes and opportunities for professional growth through ongoing support and education resources.

Qualifications

• Active Minnesota PTA license.
• Strong therapeutic exercise and functional mobility skills.
• Excellent patient and caregiver communication.

Responsibilities

• Provide one-on-one Physical Therapy treatments in patients' homes.
• Carry out individualized treatment plans.
• Provide therapeutic exercise and strengthening.
• Perform gait and mobility training.
• Work on balance, coordination, endurance, and transfers.
• Educate patients and caregivers on safe mobility techniques.
• Help patients build confidence with functional activities.
• Monitor patient response and tolerance to treatment.
• Identify and communicate changes in patient condition.
• Report progress to the supervising Physical Therapist.
• Collaborate with nurses, OT, SLP, physicians, and interdisciplinary clinicians.
• Complete accurate and timely clinical documentation.
