Job summary

CenterWell Home Health seeks a dedicated Physical Therapist Assistant to deliver in-home therapy, under the supervision of a Physical Therapist, helping patients regain strength and independence.

You will use evidence-based techniques, document care, and participate in ongoing education. Requires state PTA license, valid driver's license, CPR, and effective teamwork in a geriatric care setting.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social, and biological changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapies, therapeutic massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply issues, or staff or client changes.
• Apply infection control concepts and universal precautions in patient care activities.
• Use your skills to make an impact.
