Job summary

Metro Physical & Aquatic Therapy is seeking a full-time In-Home Physical Therapist to provide patient-centered care across Eastern Suffolk County, including Medford and Patchogue.

As a licensed NY Physical Therapist, you will evaluate home-based cases, develop personalized plans of care, and coordinate with physicians, patients, and families. Travel within the territory and a $95,000–$110,000 salary with a $3,000 sign-on bonus are offered, along with comprehensive benefits.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed Physical Therapist in New York State.
• Home health experience preferred, not required; mentorship provided.
• Ability to travel to patients' homes within the geographic area.

Responsibilities

• Accept and treat patients within the designated geographic area.

Responsibilities

Accept and treat patients within your designated geographic area

Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans

Develop a plan of care and treatment goals in consultation with the physician, patient, and family

Plan, prepare, and implement customized therapy programs to restore function and promote independence

Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques

Provide timely and accurate documentation for each visit

Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning

Deliver care in compliance with agency policies and professional practice standards

Graduate of an accredited Physical Therapy program

Licensed Physical Therapist in the state of New York or eligible for licensure

Home health experience is preferred but not required — we will provide you with a mentor to help with documentation and compliance

Ability to travel to patients’ homes within the assigned geographic territory

The anticipated base salary range for this position is $95,000–$110,000 annually. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.

This position is eligible for a $3,000 sign-on bonus. Additional terms and conditions may apply.
