Job summary

Metro Physical & Aquatic Therapy is seeking a motivated full-time Home Care Physical Therapist to provide in-home care across a designated geographic area in Eastern Suffolk, NY.

The ideal candidate is a licensed Physical Therapist in New York (or eligible) with the ability to travel to patients' homes, and a willingness to mentor new staff on documentation and compliance. All levels of experience are welcome with training provided.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

The anticipated base salary range for this position is $85,000.00-$100,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
