Job summary

Metro Physical & Aquatic Therapy is seeking a motivated full-time Home Care Physical Therapist to provide in-home care across Suffolk County, including Lindenhurst and Dix Hills. Base salary ranges from $90,000 to $110,000 per year, with full benefits and generous PTO.

All experience levels are encouraged to apply; a mentor will be provided for documentation and compliance. The role emphasizes patient-centric care, modern documentation tools, and autonomy to see patients 7 days a week within

Qualifications

• Licensed Physical Therapist in NY or eligible.
• Home health experience preferred.
• Willingness to travel to patients' homes within geographic area.
• In-home care experience desirable but not required; mentorship provided.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs and develop individualized home-based therapy plans.
• Develop a plan of care in consultation with physician, patient, and family.
• Plan, prepare, and implement customized therapy programs to restore function and promote independence.
• Instruct family members and caregivers on therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate with patients and interdisciplinary team, especially about discharge planning.
• Deliver care in compliance with agency policies and professional standards.

Responsibilities

Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques

Provide timely and accurate documentation for each visit

Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning

Deliver care in compliance with agency policies and professional practice standards

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated base salary range for this position is $90,000.00-$110,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
