Metro Physical & Aquatic Therapy is seeking a motivated Home Care Physical Therapist to provide in-home services on Long Island. This full-time role offers a competitive salary and comprehensive benefits.

Licensed NY PT required; travel within geographic area; 1099 and W-2 options, generous PTO, and opportunities for growth.

Qualifications

• Licensed to practice as a Physical Therapist in New York or eligible
• NY PT license required; home health experience preferred
• Able to travel to patients' homes within geographic territory

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

• Compensation: USD 85000 - USD 100000 - yearly

Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated base salary range for this position is $85,000.00-$100,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
