Job summary

Responsibilities

• Attend orientation/training upon hire & ongoing per policy.
• Document/write activity reports on each client assignment.
• Follow agency policies, confidentiality, and HIPAA guidelines.
• Promptly report changes to Administrator/designee.
• Maintain communication with client, family and team members.
