Job summary

Qualifications

• High school diploma or GED required; must be at least 18.
• At least 1 year home health aide experience preferred.
• CPR certification required.
• Knowledge of safety and infection control in healthcare.
• Must be a licensed driver.

Responsibilities

• Provide personal care and supportive services to PACE participants.
• Assist with activities of daily living and maintain a safe home environment.
• Observe and report changes to clinical staff.
• Collaborate with Interdisciplinary Team per plan of care.
