Qualifications

• Master's Degree in Speech Language Pathology required.
• Applicable state license for Speech Language Pathology required.
• BLS required per policy guidelines.

Responsibilities

• Assesses and documents patient's medical, mental and emotional needs at admission and on an on-going basis.
• Develops and implements a plan of care to meet patient and family needs, including discharge planning and resource utilization.
• Provides patient evaluation and care planning for speech-language pathology in the home setting.
• Provides clinical leadership to support autonomous, evidence-based therapy in the patient’s home.
