Seamen’s Society for Children and Families is seeking an in-home support specialist to promote positive parent-child interactions and provide case management linking families to community resources.

The role involves field visits, home assessments, and advocacy to help families access entitlements, housing connections, and other services while coordinating with schools and health professionals.

Qualifikationen

• HS Diploma or GED required.
• Basic computer literacy preferred.
• Work hours vary based on program needs.

Aufgaben

• Conduct field visits and escort families to service providers; medical appointments, Public Assistance centers, Medicaid offices and housing services.
• Conduct home visits per ACS FTR requirements to assess family needs and child safety.
• Advocate and assist families with connecting to community resources and referrals.
• Provide support services with casework focusing on goals, family relations, and parenting.
• Obtain collateral documents from medical, treatment and academic programs.
• Work with parents to educate on parenting and home management skills.
• Attend required trainings and meetings and conduct chart reviews monthly.
• Complete case work documentation in logs, PROMIS, CONNECTIONS and other systems.
