Job summary

RVNAhealth is seeking compassionate, motivated Physical Therapist Assistants to join our In-Home Outpatient Rehabilitation team. In this role, you’ll deliver skilled outpatient PT services in patients’ homes and senior living communities, providing one-on-one care that keeps people independent and safe.

You’ll work under a licensed PT, follow Medicare Part B, and collaborate with a multidisciplinary team. This position offers territory-based assignments across Connecticut and opportunities for

Qualifications

• Associate degree from an accredited Physical Therapist Assistant program.
• Current Connecticut Physical Therapist Assistant license.
• Outpatient, home health, or rehabilitation experience preferred, but not required.
• Strong communication, organization, and interpersonal skills.
• Ability to work independently while collaborating with a multidisciplinary team.
• Reliable transportation and a valid driver's license.

Responsibilities

• Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist.
• Implement individualized treatment plans designed to improve strength, balance, mobility, endurance, and functional performance.
• Assist patients with gait training, transfers, therapeutic exercise, and functional mobility.
• Educate patients and caregivers on home exercise programs, safety strategies, and adaptive techniques.
• Monitor patient progress and communicate clinical updates to the supervising Physical Therapist.
• Collaborate with physicians, therapists, and other healthcare professionals to support optimal patient outcomes.
• Complete timely, accurate electronic documentation in accordance with Medicare Part B and agency standards.
• Deliver compassionate, patient-centered care that promotes long-term wellness and independence.
