Job summary

Qualifications

• Current Connecticut PTA license.
• Graduate of an accredited Physical Therapist Assistant program.
• Valid driver’s license and reliable transportation.
• Strong communication, organizational, and documentation skills.
• Ability to work independently in the home setting.

Responsibilities

• Providing skilled physical therapy treatment in patients’ homes.
• Carrying out the established PT plan of care.
• Communicating patient progress and changes in status to the supervising PT.
• Completing timely and accurate documentation.
• Providing patient and caregiver education as appropriate.
• Maintaining communication with the rehabilitation team regarding patient needs and progress.
