FOX Rehabilitation is growing in Sunnyvale, CA, and seeks a licensed Occupational Therapist to deliver one-on-one care to older adults in homes or Senior Living Communities. The role emphasizes autonomy, flexible scheduling, and meaningful outcomes through the Geriatric House Calls model.

You will evaluate, plan care, and deliver evidence-based treatment, documenting in FOX’s EMR and collaborating with an interdisciplinary team.

Qualifications

• Valid Occupational Therapy license in CA or eligibility to obtain
• Degree from an accredited Occupational Therapy program
• Experience in home health or geriatric care preferred

Responsabilités

• Provide one-on-one Occupational therapy to older adults in homes or senior living locations
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that improve mobility, independence, and quality of life
• Document effectively in FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
