Powerback Rehabilitation is seeking an Occupational Therapist to provide outpatient in-home services for older adults, in patients' homes or in assisted living settings across Avon, Simsbury and West Hartford, CT. The role emphasizes functional ADL training, mobility, and home safety.

You'll collaborate with a multidisciplinary rehab team, supervise assistants as allowed, and adapt plans to fit individual needs.

Qualifications

• Bachelor's degree in an ACOTE-accredited OT program.
• Initial NBCOT certification/registration.
• Valid OT license in the state of practice.
• Valid driver's license or reliable transportation.

Responsibilities

• Provide outpatient OT services in home and facility settings.
• Supervise assistants and students as allowed.
• Maintain schedule, documentation, and billing compliance.
• Collaborate with rehab team and physicians to plan therapy.
• Adapt care plans to meet individual patient needs.
• Ensure safe environment and maintain equipment.
