Job summary

Amazing Care, Inc. is seeking an Occupational Therapist to provide in-home, individualized therapy to adult clients in the Greeley/Loveland area. The role supports independence and daily function with client-centered care in a home setting.

The candidate will develop treatment plans, perform assessments, educate patients and caregivers, and document care while collaborating with a coordinated clinical team.

Qualifications

• A current Occupational Therapist license in Colorado.
• Experience in home health or a related area.
• Great communication and people skills.
• Skills in managing electronic medical record documentation.
• A flexible and adaptive approach to cater to the unique needs of each client.

Responsibilities

• Develop and implement individualized treatment plans.
• Conduct assessments using standardized tools.
• Provide hands-on, functional training for ADLs, mobility, and safety.
• Educate patients and caregivers on home programs and adaptive equipment.
• Maintain accurate and timely clinical documentation.
• Collaborate with other clinicians as part of a coordinated care team.
