Qualifications
- Colorado Occupational Therapist license is required.
- Proven experience in home health or related field.
- Excellent communication and interpersonal skills.
- Proficiency with electronic medical record documentation.
- Flexible and adaptive approach to meet diverse client needs.
Responsibilities
- Develop and implement personalized treatment plans.
- Conduct evaluations using standardized tools and clinical expertise.
- Provide therapy focused on ADLs, mobility, safety, and independence.
- Educate patients and caregivers on home programs and adaptive equipment.
- Maintain timely and accurate clinical documentation.
- Collaborate with multidisciplinary team for coordinated care.
