Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to provide one-on-one care in patients' homes and senior living communities in Jackson Heights, NY. You’ll build treatment plans, conduct evaluations, and deliver evidence-based therapies that improve mobility and independence.

Qualifications

• Valid OT license in NY or eligibility to obtain.
• Degree from an accredited OT program.
• Current CPR certification and EMR proficiency.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver sessions supporting mobility, independence and function.
• Document effectively in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
