FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver one-on-one geriatric care in homes and senior living communities in Middle Village, NY. You will evaluate, develop individualized plans, and provide evidence-based treatments while coordinating with an interdisciplinary team.

Qualifikationen

• Valid OT license in NY or eligibility to obtain license.
• OT degree from an accredited OT program.
• Proficient with EMR systems.
• CPR certification

Aufgaben

• Provide one-on-one occupational therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and safety.
• Document effectively in FOX’s EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers and staff to promote aging in place.
• Provide telehealth/virtual care when appropriate.
