Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver one-on-one care to older adults in their homes and senior living communities in Holmdel, NJ. You will create and manage your treatment schedule, perform evaluations, develop plans of care, and deliver evidence-based therapy to promote independence and mobility.

Qualifications

Valid Occupational Therapy license in the state of practice.
Proficiency with electronic medical records (EMR).
Current CPR certification.

Responsibilities

Provide one-on-one occupational therapy to older adults in homes or senior living settings.
Perform evaluations, develop care plans, and deliver evidence-based treatment.
Deliver therapy to improve mobility, independence and functional skills.
Document in EMR per regulatory standards.
Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
Educate patients and caregivers to promote safety and aging in place.
Offer telehealth/virtual care when appropriate.
