Job summary

FOX Rehabilitation seeks a licensed Occupational Therapist to provide in-home, senior living, and virtual care to older adults in Nanuet, NY.

You will perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment while documenting in FOX’s EMR and collaborating with an interdisciplinary team.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in their homes or communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists as a team.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
