Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver in-home and senior living care in Monroe, NJ. You will perform evaluations, design personalized care plans, and provide evidence-based therapy, helping older adults regain mobility and independence.

Qualifications

• Valid Occupational Therapy license in NJ or eligibility to obtain
• Degree from an accredited OT program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one occupational therapy to older adults in-home or in senior living settings
• Perform evaluations and develop individualized plans of care
• Deliver therapy sessions to improve mobility, independence, and QoL
• Document in FOX’s EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs and Fitness Specialists
• Educate patients, caregivers and staff to promote safety and aging in place
• Offer telehealth/virtual care when appropriate to extend access
