Job summary

CenterWell Home Health is seeking an Occupational Therapy Assistant to support in-home rehabilitation, reporting to the Home Health Branch Administration. You will deliver supervised therapy to improve strength, mobility and independence, and assist in designing adaptive environments and activities for patients and their families.

You will work roughly 40 hours per week, with duties that include home program instruction, care coordination, and participation in discharge planning to promote

Qualifications

• Current and unrestricted OTA licensure.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Provide therapy services planned, delegated and supervised by the Occupational Therapist in accordance with the patient's Plan of Treatment.
• Assist in implementing vocational/education programs and activities established by the Registered Occupational Therapist.
• Design/adapt equipment and living environments to restore and enhance function and independence.
• Provide therapeutic treatment and instruction to improve/restore strength, coordination, ROM and function.
• Document observations in patient records and prepare progress reports as required.
• Instruct patients and families on home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning with the interdisciplinary team.
• Coordinate community reintegration activities to support physical reconditioning and psychological adjustment.
