The role emphasizes coordination with the care team, evidence-based practices, and documentation to support CMS guidelines, with a commitment to compassionate, personalized care in patients' homes.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsabilités

• Provides service within the scope of practice as defined by state laws.
• Evaluates patient functional status and CMS documentation needs; consults physician on therapy plan of care.
• Observes, records, and reports patient responses to treatment and condition changes to supervisor/physician.
• Conducts patient assessments using functional ability tests.
• Ability to work in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
