LHC-Illinois Home Health Care is seeking an Occupational Therapist to assess and address patient needs related to functional status, ADLs, fine motor skills, home assessments and adaptive equipment. You will work within your scope of practice, coordinate with the health care team, document CMS data, and contribute to care plans of care.

Qualifikationen

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and access to a vehicle or public transportation.

Aufgaben

• Provides service within the scope of practice as defined by state laws and plan of care, coordinating with the health care team.
• Evaluates patient’s functional status and CMS documentation; consults with physician on therapy plan of care.
• Observes, records, and reports patient response to treatment and changes in condition.
• Conducts patient assessments evaluating functional level using appropriate tests.
