Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist to help patients regain strength and independence in their homes. You will work with physicians and the care team to design and implement rehab plans, assess functional needs, and document progress.

A valid OT license and CPR certification are required, with preferred Home Health experience. This role involves travel to patients’ homes, collaboration with a multidisciplinary team, and participation in care coordination and

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience preferred.

Responsibilities

• Assess daily living/work-related skills and develop retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function in patients’ homes to aid physicians.
• Collaborate with physicians and care teams to update the Plan of Care.
• Provide therapeutic treatment to improve strength, range of motion and function.
• Document patient progress and adjust retraining programs as needed.
