Job summary

Mercy Medical Center Merced is seeking an Occupational Therapist to provide essential in-home therapy under physician orders. You will evaluate home safety, assess functional needs, and design individualized treatment plans to promote independence for patients across ages.

You will conduct ongoing evaluation and education for patients and families, collaborate with physicians, and use therapeutic equipment to support safe, effective care in the home setting.

Qualifications

• Licensed occupational therapist with CA licensure.
• CPR certification required.
• Experience delivering in-home therapy services.
• Strong communication for patient and family education.

Responsibilities

• Evaluate home safety and functional status of patients.
• Develop and implement individualized home-based OT plans.
• Coordinate with physicians to follow orders.
• Document progress and adjust treatment plans.
