Job summary

Fox Rehabilitation in Walton, KY is seeking licensed Occupational Therapists to deliver one-on-one care to older adults in homes or senior living communities, using the Geriatric House Calls model. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes with opportunities to incorporate telehealth.

You will perform evaluations, develop plans of care, work with an interdisciplinary team, document in FOX EMR, and educate patients and staff to promote safety and aging in

Qualifications

• Valid OT license in the state(s) of practice, or eligibility to obtain
• Current CPR certification
• Basic computer and EMR proficiency

Responsibilities

• Provide one-on-one Occupational therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility
