Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver one-on-one care in Washington Court House, OH, in private residences or senior living settings. The role emphasizes autonomy, flexible scheduling, and strong patient relationships within the Geriatric House Calls model.

You will perform evaluations, create individualized care plans, and provide evidence-based therapy to improve mobility and independence, with optional telehealth to extend access and continuity of care.

Qualifications

• Active OT license in Ohio or eligibility to obtain
• Degree from an accredited OT program
• CPR certification and EMR proficiency
• Strong documentation skills in EMR system

Responsibilities

• Provide one-on-one OT to older adults in homes or Senior Living Communities
• Perform evaluations, develop individualized plans of care, deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence and function
• Document effectively in FOX’s EMR per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
