Job summary

Nightingale Home Healthcare of Minnesota Inc is seeking a full-time Occupational Therapist (OT) to serve the Minneapolis/Saint Paul area. You will evaluate and treat patients in their home, designing therapy plans to restore independence and coordinating with the rehab team.

Qualified candidates are MN-licensed OTs with CPR, TB test, and a valid driver's license. Orientation at Minnetonka is required, with competitive per-visit pay and full-time benefits available.

Qualifications

• Licensed Occupational Therapist (OT) in MN.
• Minimum 1 year OT experience preferred.
• Experience with OASIS and WellSky (Kinnser) strongly preferred.
• Current TB test (within last 12 months) and CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to attend a 2–3 day orientation at Minnetonka

Responsibilities

• Provide effective patient care using approved therapy techniques and procedures.
• Evaluate patients’ condition, function level, and treatment goals.
• Develop individualized treatment plans and update as needed.
• Supervise COTAs and HHAs per care plan requirements.
• Document treatment notes and progress in the patient’s chart daily.
• Assist patients and families with home exercise programs and adaptive equipment needs.
• Recommend and evaluate adaptive equipment for the home setting.
• Maintain HIPAA compliance and company policies.
• Coordinate total patient care with the healthcare team.
• Complete discharge summaries and obtain orders when needed.
• Submit patient documentation and visit records in a timely manner
