Job Title: Occupational Therapist (OTR/L) | Build Independence, One Home at a Time

Schedule: Full-Time | Monday–Friday

Compensation: $80,000–$150,000 annually

To Stand Out: Email Nikky Vogelgesang at nicole.vogelgesang@hhcare.net with a brief summary of your OT experience and what you're looking for in your next opportunity.

Your Patients Don't Need Another Appointment. They Need Someone Who Understands Their Life.

Occupational therapy is most effective when it's practical.

Instead of asking patients to imagine how they'll function at home, you'll see it for yourself.

You'll identify barriers, recommend solutions, and create treatment plans that fit their routines, goals, and environment. Whether you're helping someone safely prepare a meal, return to dressing independently, or prevent future falls, your work creates confidence that lasts long after the visit ends.

Why Occupational Therapists Thrive in Home Health

  • Deliver truly personalized therapy in each patient's home.
  • Focus on functional outcomes that improve everyday living.
  • Build meaningful relationships with patients and caregivers.
  • Enjoy the freedom to manage your own schedule while maintaining strong clinical support.
  • Work alongside experienced nurses, therapists, physicians, and clinical leaders who value collaboration.

Why Home Health Care, Inc.

For over 30 years, Home Health Care, Inc. has been one of Minnesota's leading home health providers because we invest in the clinicians who make exceptional care possible.

When you join our team, you'll enjoy:

  • Competitive compensation with excellent earning potential.
  • Free access to our MedBridge Education Hub for continuing education, certifications, and professional development.
  • Licensure reimbursement when your license is due for renewal.
  • Cell phone reimbursement for eligible clinicians.
  • Comprehensive medical, dental, vision, life insurance, disability coverage, PTO, paid holidays, and 401(k).
  • Experienced Clinical Managers who support your success in the field.
  • A stable, locally owned organization with more than three decades of clinical excellence.

What You'll Do

  • Evaluate patients in their homes and develop individualized occupational therapy treatment plans.
  • Improve independence with activities of daily living, home safety, upper extremity function, cognition, and adaptive techniques.
  • Recommend home modifications and adaptive equipment that support long-term independence.
  • Educate patients and caregivers on strategies that improve safety and function.
  • Collaborate closely with physicians and the interdisciplinary care team to achieve outstanding patient outcomes.

What We're Looking For

  • Active Minnesota Occupational Therapist (OTR/L) license.
  • Strong clinical reasoning and patient-centered care philosophy.
  • Excellent communication and organizational skills.
