Job summary

Senior Home Health Care is seeking an Occupational Therapist to join our therapy staff. The role offers part-time or full-time positions servicing clients in the metro area, with a robust benefits package including PTO, health/dental/vision, 401(k), and paid holidays.

FT employees receive comprehensive benefits; the position requires Minnesota licensure, BLS, and a valid driver's license. Previous home health care experience is preferred.

Qualifications

• Two years' experience in occupational therapy required.
• Graduate of an accredited OT program.
• Current Minnesota OT license.
• American Heart Association BLS required.
• Driver's license and reliable vehicle with insurance.

Responsibilities

• Provide OT services within plan of care and state guidelines.
• Lead patient in therapeutic, creative, and self-care activities to improve function and safety.
• Observe, record, and report patient responses to treatment to supervising nurse/physician.
• Instruct patient, family, and care team in therapy activities to support care.
• Visit patients per plan of treatment and complete progress notes for each visit.
• Complete evaluations and develop OT plan of care; report outcomes within 24 hours.
• Supervise COTA and prepare discharge summaries at least every 60 days.
• Conduct patient assessments using functional ability tests.

JOB DESCRIPTION

• Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Follows the plan of care by leading the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observes, records, and reports to the supervising nurse and the physician the patient's response to treatment and changes to the patient's condition
• Instructs patient, family, caregiver, and participating members of the health care team in the areas of therapy in which they can participate to assist the patient.
• Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes
• Completes all patient evaluations and develops the OT plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to supervising nurse within 24 hours of the evaluation.
• Provides supervision of COTA.
• Prepares a written discharge summary and/or a written summary report on the patient's condition at least every 60 days.
• Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.

QUALIFICATIONS:

• Minimum two (2) years of experience in occupational therapy.
• Graduate of an accredited occupational therapy curriculum.
• Current license to practice as an Occupational Therapist in Minnesota.
• American Heart Association approved BLS.
• Current valid driver's license required.
• Reliable automobile and valid auto insurance
• Previous Home Health Care Experience strongly preferred.

Job Type: Full-time

Pay: $85,000.00 - $95,000.00 per year
