Job summary

Optimal Home Care is seeking an Occupational Therapist (OT) to provide high-quality OT services to patients in the Colorado area. The OT will deliver care per AOTA standards, ensure patient safety and privacy, and collaborate with clients and a multi-disciplinary team to meet goals.

Responsibilities include evaluating patients, developing Plans of Care with physician orders, and documenting progress and outcomes to support ongoing care transitions within the home health setting.

Qualifications

• Graduate of ACOTE-accredited OT program.
• Must be licensed in Colorado and in good standing.
• Ability to communicate effectively with clients; documentation skills.
• Strong interpersonal and collaborative skills.

Responsibilities

• Provide occupational therapy services in compliance with AOTA standards and regulatory requirements.
• Ensure patient safety and HIPAA compliance in all notes and interventions.
• Collaborate with clients, families, and interdisciplinary team to develop goals.
• Develop and implement Plan of Care based on evaluation, goals, and MD orders.
• Document outcomes, progress, and transitions between service levels.
