Job summary

Optimal Home Care is seeking an Occupational Therapist (OT) to provide high-quality home health OT services to patients in Colorado. The OT will ensure compliant service delivery, patient safety, and coordination with the care team.

Responsibilities include evaluation, Plan of Care development, interdisciplinary collaboration, and documentation to meet regulatory and payer requirements. Colorado licensure and ACOTE-accredited OT program completion are required for this role.

Qualifications

• Graduate of an Occupational Therapy program accredited by ACOTE or its predecessor organizations.
• Must be currently licensed and in good standing in the state of Colorado.
• Must be able to communicate in the language of the client and document client related activity as appropriate.
• Possesses adequate and effective written and verbal communication skills.
• Possesses adequate and effective interpersonal skills.

Responsibilities

• Provides occupational therapy services in compliance with AOTA standards, policies, and guidelines, and applicable laws and payer requirements.
• Maintains patient safety and dignity while protecting PHI and HIPAA compliance.
• Develops and implements OT interventions based on patient goals and clinical reasoning.
• Documents Plan of Care and ensures MD approval within required time frames.
• Collaborates with client and interdisciplinary team to evaluate daily life participation.
• Coordinates care with other agency disciplines to ensure continuity of care.
• Supervises or coordinates with COTA and CNA as required by regulations.
